When an eye examination reveals more

Shahni has identified serious eye-health concerns during routine and symptom-led examinations, including extensive retinal haemorrhaging with optic nerve swelling and a separate case involving progressive central vision changes.

Both patients were referred urgently for ophthalmological assessment so the findings could be investigated and managed appropriately.

The retina is the light-sensitive tissue at the back of the eye. Changes may sometimes be detected during a comprehensive eye examination before noticeable symptoms develop.

Case 1 · Urgent ophthalmology assessment

Retinal haemorrhages and optic nerve swelling identified

A patient attended City Optics Kotara after experiencing a gradual decline in vision in one eye over approximately two months.

During the examination, Shahni identified extensive retinal haemorrhages and swelling of the optic nerve in the affected eye. She also recorded markedly raised pressure in the other eye, despite that eye being asymptomatic.

The retinal findings were suspicious for a central retinal vein occlusion, while the raised eye pressure was a separate concern requiring prompt assessment. Shahni arranged an urgent referral to a hospital ophthalmology service for further investigation and management.

This case illustrates why an eye examination considers both eyes and more than the symptom that first prompted the visit. Important findings may also be present in an eye that feels normal.

Case 2 · Urgent specialist referral

Gradual central vision changes prompted urgent referral

A patient attended City Optics Kotara for a routine glasses update after noticing a gradual decline in vision. Their previous eye examination had been several years earlier.

Testing identified reduced central vision, colour-vision loss and visual distortion in one eye, with some distortion also detected in the other eye. Examination of the retina revealed an area of geographic atrophy in the more affected eye, suspicious for age-related macular degeneration. Cataracts and other anterior eye changes were also noted.

Shahni arranged an urgent referral to an ophthalmologist for further assessment. The referral did not claim a confirmed diagnosis or outcome.

This case shows why gradual changes in central vision should be assessed rather than assumed to be a routine prescription change. A comprehensive examination can help distinguish between refractive needs and eye-health findings requiring specialist care.
