Real cases from our Nepean Village clinic
Sharon has identified important health findings during routine eye examinations, including bilateral optic nerve swelling in a patient who had no symptoms and a small cholesterol plaque within a retinal blood vessel.
Both findings prompted medical investigation beyond the eye examination. In one case, the subsequent work-up identified a previously undiagnosed heart condition.
The retina is the light-sensitive tissue at the back of the eye. Changes may sometimes be detected during a comprehensive eye examination before noticeable symptoms develop.
Case 1 · Bilateral optic nerve swelling
A patient attended City Optics Nepean Village for a routine eye examination and did not report any concerning visual or neurological symptoms.
During the fundus examination and OCT imaging, Sharon observed swelling of both optic nerves consistent with bilateral papilloedema. Papilloedema can be associated with increased pressure inside the skull, so Sharon referred the patient to their GP for further investigation to determine the underlying cause.
The case demonstrates why an eye examination can remain important even when vision appears satisfactory and the patient feels well. Changes visible at the back of the eye may provide an early indication that further medical assessment is required.

Case 2 · Medical work-up recommended
Another patient attended City Optics Nepean Village for a routine eye examination without a known retinal concern.
During the examination, Sharon incidentally identified a Hollenhorst plaque within a retinal blood vessel. A Hollenhorst plaque is a small cholesterol embolus that may be associated with disease elsewhere in the cardiovascular system, even when it is not affecting the patient’s vision.
Sharon referred the patient to their GP for cholesterol testing and assessment of their stroke and cardiovascular risk. The subsequent medical work-up identified a previously undiagnosed heart condition, allowing the patient to receive appropriate medical attention.
This case highlights how examining the retinal blood vessels can sometimes reveal signs relevant to a patient’s broader health—not only their eyesight.
